Please provide the information listed below.  Seats are reserved on a first-come / first-serve basis.  If you choose to send us a check in lieu of paying online with a credit card we will hold your seat for 7 days from the date we receive your registration form.  In the event that payment is not received within that time frame, your seat will be relinquished to the next person on the waiting list.

In the even that we are significantly over subscribed we will consider adding a second session of this seminar.  If the current date conflicts with another event on your calendar please feel free to fill out the registration form and use the comments section to indicate other dates that might work for you.  We will do our best to meet your needs.

Prior to the event we will forward a survey form to you via e-mail.  The information on that form will be used to tailor this session to meet the specific needs of this audience.  Therefore it is very important that you respond to that e-mail as quickly as possible.

Should you need to cancel it is important that we receive your cancellation as early as possible.  There are no refunds if your cancellation is received less than 14 days prior to the event.  We will, however, credit your payment to a seat at one of our future events.

You may purchase this seminar either with or without bus transportation.  We use PayPal for our online payments.  You do not need a PayPal account to use this service.  When clicking one of the payment buttons below you will be redirected to PayPal's secure server.  If you have a PayPal account you will login on the right hand side of the page.  If you are not a PayPal member there is a credit card option in the lower left hand corner of the page. 

You are also welcome to send us a check.  Click the button below to download a transmittal form that includes mailing information.  Please remember that we will hold your seat for 7 days from the date of registration.  If your check is received later than that you will risk losing your seat.
